Win the club lead, go to the dummy in spades, and lead a heart from dummy. You must hope that either East holds the ace and king or that East holds the king and plays low on the heart lead. In the latter case, West wins the trick and you can ruff out the king of hearts to establish a pitch for one of your clubs.
