It's 50-50 as to whether the queen is on your
right or left. But the correct play is to cash the
ace and king, and then run dummy's jack if it's
not covered.

This gives you the extra chance of singleton or
doubleton queen with East. In addition, there is
no reason to think East would cover with the queen,
unless East is new to the game

 

After the jack of
hearts is led and
East fails to cover,
it's clearly right to
let it ride. East may
hold four or five
hearts to the queen.
Trumping the third
round of hearts wins
only when West
has precisely
three to the queen.
